The Darkwraith probably got inside because of Omission Mode, an optimization feature where all the creatures your camera isn't facing are given reduced animation frames, because of this the game will determine where they're going and parodically teleport them there (usually small distances of a few feet). When you run past enemies they charge straight into the fog gate, and sometimes while you're not looking at the fog gate, the Omission Mode will screw up their position and teleport them through the door.

amazed that a darkwraith made it into the abyss watcher's room. were they "pushed" into the room or coaxed inside? normally they stay several feet away from the door.
