Geralt was knighted by Meve of Lyria after his famous defense of the bridge across Yaruga agains Nilfgardians. She knighted him as "Geralt of Rivia" because the bridge was in the area named Rivia. (books)
(Fun fact.. he was known as Geralt of Rivia from start.. Vesimir gave him that addition to look better.. for customers.. )
This is most likely his second knighting by "Lady of the Lake" - goddess.. so.. witcher and twice the knight!
