I did one of these back in 2014. Healthy Wage or Healthy Wager. My company sponsored it as a team weght loss challenge. You could bet on yourself. I think I had to lose 15% for my bet. Bet $30 and got around $100 when I met the goal, but you had to keep it off for like 6 months to get the payout.
